Privacy Notice for Patients

 

How we use your personal information

This notice tells you why Your Chiropractor Limited collects information about you and how this information may be used.

Chiropractor Antony Honeker, who provides your care, maintains records about your health and any treatment or care you have received here or previously. These records help provide you with the best possible healthcare. Your requesting treatment and our agreement to provide that care constitutes a contract. We have a legitimate interest in collecting relevant information about you, because without it we couldn’t do our job effectively and safely. We also have a legal requirement to keep good treatment notes as laid out by our government appointed regulator the General Chiropractic Council (GCC). Our records are electronic and on paper and we use a combination of working practices and technology to ensure your information is kept confidential and secure. Records which this clinic holds about you may include the following information:

  • Details about you, such as your address, contact details, previous medical history and previous investigations.
  • Any contact that the clinic has had with you, such as appointments, clinic visits, or advice given over the phone or by email.
  • Notes about your health.
  • Details about your treatment and care.
  • Relevant information from other health care professionals.

Information may be used within the clinic for clinical audit purposes to monitor the quality of the services we provide. Information may also be used for statistical or research purposes. Where we do this, we take strict measures to ensure that individual patients cannot be identified.

How do we maintain the confidentiality of your records

We are committed to protecting your privacy and will only use information collected lawfully in accordance with:

  • General Data Protection Rules (GDPR) 2018
  • Human Rights Act 1998
  • Common Law Duty of Confidentiality
  • General Chiropractic Council (GCC) Code of Conduct

Here at Your Chiropractor Limited, we have a legal obligation to keep information about you confidential. All of your information is held securely on our premises and within ‘Cliniko’, a secure GDPR compliant password-protected cloud-based patient management system (www.cliniko.com).

Who do we share your information with?

We only ever pass on information about you to others, if there is a genuine need for it and you have given your consent. This may be your GP, dentist or other health care professionals as appropriate, a solicitor or for court proceedings.

We will not disclose any information about you to any third party without your permission or in case of a child’s information the parental consent, unless there are exceptional circumstances (i.e. life or death situations), where the law requires information to be passed on and/or in accordance with the Caldicott principles.

Access to your personal information

You have a right under the General Data Protection Rules 2018 to request access to view or obtain copies of what information Your Chiropractor Limited holds about you and to have it amended should it be inaccurate. Your request must be made in writing to the clinic (details below). You may need to give us proof of name (Photo ID) so that your identity can be verified. There is no charge for copies of your file, and we are required to respond to your request in normal circumstances within 1 month.

Retaining your personal data

Whilst you are a patient / client of us we will continue to store and use your personal data. We have a legal requirement to keep your medical records for a minimum of 8 years from the date of your last visit to us (or for minors, up to their 25th birthday). After this time, we may continue to store your medical records indefinitely in order that we can provide you with the best possible care should you need to see us at some future date. However, should you wish, you can request for your data to be removed from our system once the legally required storage period has elapsed. Limited information may be retained within our accounts systems indefinitely to maintain the integrity of the data.

Should you have any concerns about how your information is managed at the clinic, please contact Your Chiropractor Limited in the first instance (details below). If you are still unhappy following a review by the Data Controller, you can then complain to the Information Commissioner’s Office via their website (www.ico.org.uk).

Change of details

It is important that you tell us if any of your details such as your name or address have changed, or if any of your details such as date of birth are incorrect, in order for us to correct your records.

Notification & Data Controller

Your Chiropractor Limited is the data controller, meaning that it determines the processes to be used when using your personal data, and it is registered with the Information Commissioner’s Office.
